Dear Student-Athlete:

On behalf of the Sports Medicine Staff, we would like to welcome you to the University of North Dakota. Before you participate, your coach and/or athletic trainer will contact you with instructions on how to properly fill out the necessary paperwork, medical history information, and copy of your health insurance card. Pre-participation exams will be arranged for teams when they arrive on campus. No outside physicals will be accepted. All necessary paperwork must be completed before the student-athlete will be allowed to participate in any practice or conditioning activities.
