Complete this form for all your new hires and submit to Office Manager two weeks prior to your first camp.
Direct Deposit Exemption Form
Payroll highly recommends all employees use direct deposit. Only use this form if the hire has a good reason for not using direct deposit.
Activity/Event Approval Form
This form can also be completed and submitted online here. Complete this form at least two weeks prior to the beginning of your camps.
Student Break Waiver Form
Have each student working your camp complete this waiver form. Return to Office Manager.

Athletic Event Pay Form
Use this form to pay non-students, non-employees (officials, score keepers). Be sure to ask the camp worker being hired if they receive a paycheck from UND or if they have within the last year. If they do or have, they will be processed through payroll.
